Eligibility Requirements

Please provide an valid Birth Certificate as proof of age.

You must reside within the state of Arizona and provide one of the following documents as proof of residency to be eligible for enrollment.
Gas, electric, or water bills from the last 60 days include your name and service address.
Mortgage statement from the last 60 days or annual lease agreement.
Arizona Driver’s License/State ID
State, Tribal, or Federal Agency Documentation

All students must provide evidence of their immunizations on either:
Arizona official immunization form
An official school record
A healthcare provider or health department printout
An immunization booklet with all doses stamped or signed.
These forms must be stamped or signed by a healthcare provider.
Arizona allows for medical, personal, or religious exemptions. Medical exemptions must be signed by a healthcare provider. Parents seeking exemption from immunizations for medical, personal, or religious reasons must submit an exemption form. The Medical Exemption Form and the school Personal Beliefs Exemption Form is for students in K-12 grades.

Families must provide previous school records to align curriculum and grade level requirements. Please provide one of the following:
Report Card (Grades 1-8)
Transcript (Grades 9-12)
Homeschool documentation.

Arizona Virtual Schools serves students in grades K-12.

You may need to provide additional documentation if any of the following circumstances apply:
Custody order: If your legal guardianship is determined by court order, please submit a copy of the order.
Gifted evaluation: If you are requesting enrollment in a course that is ahead of or behind your student’s age-based grade level or you are requesting placement in a Gifted course, an Academic Services Counselor will contact you to request a report card, Prior Academic History Form, and/or state test scores.
Special Programs Records: If your student falls under a special program (e.g., Special Education, 504, ESL), please provide the most up-to-date paperwork so we can verify and begin providing the appropriate services.
You will submit all documentation during the enrollment process.
